Description of:
On July 15, 2016 at approximately 1445 hours Sergeant Teneyck reported to me that Trinity staff
were having prisoners sort through the potatoes and remove the ones that had maggots in them.
Myself and R.U.M Mansfield reported to the back dock of the food service building. On the back
Dock there was a overwhelming stench from the rotten potatoes. Food Service Supervisor Tyson
was supervising prisoners sorting through the maggot infested potatoes and separating them from
the good ones. I asked Warehouse employee Rye why are these rotten? He replied They have
been sitting in the warehouse in extreme heat for over a week. There was 1 pallet of the load
that did not have maggots and yellowish/white liquid seeping from the bags.
I asked F/S Tyson if we keep this pallet is it enough to provide for the meals until Monday at
noon. She stated Yes. RUM Mansfield phoned Warden MacLaren. The warden advised to have
the spoiled product removed from the institution. After the spoiled produce was removed, the
entire back dock area was scrubbed down with disinfectant and sanitized. The prisoners that were
exposed to the spoiled items were returned to their unit and allowed to shower. They were issued
clean kitchen whites when they returned to their food service assignments.

Description of Staffing Event: Hodo #275076 reported to CMO Sata that Trinity worker Mr. Brown walked into the
main kitchen and saw Hodo eating an apple. Mr Brown said to Hodo why are you eating an apple, dont you guys
eat bananas? Hodo is Non-white. This comment was made in front of Trinity worker Mrs Bradley, and other
inmates as well, both white and non-white. Many inmates became upset instantly, causing the kitchen to become
un-settling/buzzing with tension. Upon Mrs Bradley reporting this to me, I then called the Control center and Lt.
Dean came out and took statements and also helped diffuse the tension in the main kitchen.

4/6/16 update. Trinity held an internal investigation and separated Mr. Brown as a result of their findings.MDOC has assigned an AIPAS internal
investigation which is currently in progress. No further action required.
